Privacy notice
What happens to the information you send through this website. Written to be read rather than to cover me, so it is short.

This notice is a draft. The registered controller details, the retention periods and the form provider's own data terms all need confirming before launch. It is written to be accurate about how the site actually works, but it has not been reviewed by anyone qualified to sign it off.
Who is responsible for your information
Dr Yunus Akhalwaya, an independent dental practitioner registered with the General Dental Council under number 76806, practising at Together Dental, St Michael's House, St Michael's Lane, Braintree, Essex CM7 1EY. Contact: braintreereception@together.dental, or 01376 321 789.
I am an independent practitioner rather than an employee of the practice I work from, so I am the controller for the information you send me through this site. Information about NHS care you may receive at the same address is handled separately by the practice.
What this website collects
The site itself sets no cookies, runs no analytics and loads nothing from third-party servers. There is no tracking to opt out of, which is why you will not see a cookie banner.
The only information collected is what you type into the enquiry form: your name, phone number, optionally your email address, which treatment you are interested in, when you would like to be contacted, and anything you write in the message box.
Health information
If you describe a dental problem in the message box, that is health information, which data protection law treats as a special category needing extra care. Please only tell me what you need to in order to get a useful reply. Do not send photographs, records or clinical details through the form.
Why I hold it, and for how long
Only to reply to your enquiry and, if you go ahead, to arrange your consultation. Your details are not added to a mailing list, not used for marketing and not passed to anyone else for their own purposes.
Enquiries that do not lead to an appointment are deleted within TODO · Retention period · §11. If you become a patient, your clinical records are then kept for the period required of dental records, which is currently at least 11 years for an adult, and until age 25 for a child.
Who else sees it
The form is submitted through a third-party form provider, which processes the submission and sends it to me by email. That provider acts on my instructions and does not use your information for its own purposes.

Your rights
You can ask me for a copy of what I hold about you, ask me to correct it, or ask me to delete it where I am not required to keep it. Email braintreereception@together.dental and I will respond within one month.
If you are not happy with how I have handled your information you can complain to the Information Commissioner's Office at ico.org.uk, or on 0303 123 1113. You can go to them directly, but I would rather you gave me the chance to put it right first.
